Download our Online Order Form. Adobe Acrobat Reader is required to view (see below). The order form can be printed and used for mail or fax orders, or as a convenient reference. Our shipping information, order policies, and contact information are in the Order Form.

Payment through PayPal:
You can pay through PayPal using your PayPal account or with any of the cards or other payment methods PayPal accepts. It is not necessary to have an account with PayPal to pay with a credit card through PayPal.

Shipping Charges:
Our Shipping Charge is a flat $5.00, regardless of the size of the order, to any USA Zip Code, including Military, usually by Priority Mail, or to Canada by Air Mail. Shipping charge to all other countries is $10 for the first item and $5 for each additional item shipped with it.
Design Factory Privacy Policy:
No customer information is ever shared or sold to other entities. When your information is no longer needed for our files, it is destroyed.
